ProMedical Plan PHC, Inc. does one thing and only one thing….affordable health care for those who need a solution for the rising health care costs.

Founded in l998, The Pro Medical Plan offers a BASIC PLAN with a COMPREHENSIVE BENEFIT PACKAGE for Nonprofit Employers that needs cost effective solution for their employees and their families, as well as volunteers and members. 

ProMedical will furnish a scheduled on-site representative at your company for 5 employees or more for enrollment.

An additional hospitalization rider is also available for a minimal cost.

ProMedical Plan is licensed as a PHC Plan (managed care prepaid health plan) which, according to the Florida Department of Financial Services, is not subject to the Federal ERISA regulations.  This means employees that are enrolled in another health plan may elect ProMedical Plan coverage for their dependents. In addition, this allows children alone to be enrolled in the plan.

Basic Plan:

ProMedical Plan provides Primary Care Physician and Pediatrician Care through approved participating private practice physicians. (over 300 to choose from, no clinics).

Comprehensive Benefit Package:  

·         3000 Specialists at fixed co pays (no referrals required)

·         Specialist Care Visit Coordination (example: Chiropractic & Podiatry  – no visit limit)

·         Prescription drugs are provided at all major pharmacies nationwide (mail order available) 

·         Laboratory Analysis & Diagnostic Centers

·         Radiology tests (X-rays, Ultrasounds, EKGs, etc.)

·         Urgent Care centers at fixed co pays (affiliated with contracted hospitals)

·         Outpatient surgical centers (affiliated with contracted hospitals)

·         Maternity package 

·         Vision, Hearing

·         Durable Medical Equipment

·         Diabetic Supplies – free glaucometer

·         Hospital Discounts

·         Dental plan

Break down of cost

Basic Plan:

• The cost for the Basic and Comprehensive package is $58.00 (Miami/Dade, Broward, and Palm beach) for the first member and $65.00 in Central Floridafor the first memeber.  Price decreases as more members are added on the plan. 

• A co pay of $20.00 per Primary Care Physician and Pediatrician visit (only $10 for Broward, Miami Dade and Palm Beach

  counties)

All applicants are guaranteed issued!

·         No Physical examinations and lab tests are not required to be approved

·         No age, health, or weight limitations; ProMedical Plan does not deny membership for applicants with pre-existing conditions or for any other cause

·         No waiting periods

·         No maximums including doctor visits and prescription drugs

·         No social security number needed

·         No contract, month to month membership
